I think that this is the kind of teacher that every med school should have. Personally, I like how he integrates all the information that it is given across the whole carrer. As a fifth year student from another country I would like that every class of lecturio be like the classes of Dr. Raj. I do not have a major problem with the material he teaches, although sometimes he just points out to the facts and skips explanations. My major issue is with the overall demeanor of Dr. Raj while he is teaching. He tries to come across as someone cool and the George Clooney of the lecturio and it's very annoying and distracting. At least for me. For those who haven't noticed, look at his facial expression when delivering a lecture. I wish he would just lecture plane and simple without resorting to " hey you know I'm cool, right?". I try not to look at him and just stare at the slides to minimize this effect.

Great lecture given by a competent speaker. But Dr Bord let us down a little by not elaborating on a clinical teaching point and pearl of wisdom that she may have wanted to talk about in more detail. She makes several references to the subtle presentations PE can take and gives two personal examples in her introduction. She also encourages us to follow our gut instinct if PE is on our minds as a likely diagnosis. I have learnt that if you go looking for text book presentations then you are on the wrong track, and to some extent this lecture encourages us to think text book. One of the most confounding and frustrating aspects of PE is that it can present in many ways - it's a mischievous demon. For me shortness of breath is the only reliable symptom. I would like a warning given to include PE in the differential diagnosis of any patient presenting with shortness of breath. However, despite my warning PERC and Wells are a good aid for getting you out of a misdiagnosis, but you have to be thinking PE to begin with. If, as clinical educators, we pushed this point in 101 then perhaps more PEs could be picked up earlier.

I just watch videos in hypertension in cardiology playlist. When the quiz pop up, I answer them all ????% correctly. What does that mean? The explanation in the video is easy to remember. Really, if I have money, I wanna subscribe for 1 year and watch all the video.

seriously these series of lectures are the absolute worst. no explanation at all, no physiopathology, no images...just a pathologist who seems to be severely under qualified to teach a comprehensive clinical neurology lecture , reading a bunch of bullet points off the most lazily prepared power point presentation. seriously strating to wonder what the point of lecturio is. I do the q bank right after the lectures and literally nothing that is asked is mentioned in these lectures.

The answer was that ARDS following sepsis will decrease the cardiac output, and that was according to what Dr. Raj said in the lectures. Is it possible that we could also have a situation with respiratory rate increasing?

I'm a huge fan of Dr. Raj so this may be a bit biased. Dr. Raj does an excellent job of hitting the high points and his anecdotes are quite relevant. You almost feel as though you're on rounds or in the clinic and there's a patient presenting with symptoms. Video examples of the endoscopes would be awesome btw.

Good lecture. The talking is very smooth and continuous not robotic like it sometimes tends to be the case with Dr.Raj. Excellent! Good explanation of the terms and the difference between atopic and contact dermatitis!
Some more elaboration on treatment options, including non-pharmacologic and the role of allergen testing (should you do it or not?) would be a good addition.

I like his explanation and also him demonstrating techniques or anatomic locations on his own body, however the diction feels very unnatural. This is in all the videos. Can that be improved? It becomes grating to listen to over time and speeding up the video doesn`t do much to improve it.

Also what about the impact the window period has and the new for follow up testing? Finally it would have been good to discuss what is considered to be as treatment failure with HAART.

Management section can be improved

Very comprehensive discussion. It would have been worthwhile to talk about criteria for starting treatment for chronic Hep B (e.g DNA count vs symptoms and livers functions). Also unclear if treatment of HepB is curative. The hepatitis talks did not mention genotypes of Hep C at all which is important for treating the condition, particularly in countries where access to direct acting antivirals is not available. Finally the talks didn't mention what percentage of patients with chronic Hep B and C go on to develop cirrhosis and hepatocellular carcinoma.
What about parasitic causes?

Moreover, a discussion on what constitutes medication overuse headache is warranted given the ease of access to OTC analgesics. Finally an opportunity has been missed to discuss if which circumstances you would consider the use of prophylactic therapies

It would have been great to list the most common agents that could potentially lead to liver toxicity aside from paracetamol, incl phenytoin, statins, amiodarone, methotrexate. Also not sure if this would be discussed in the Emergency Medicine talks but actually working up the patient and what to do from a monitoring point of view once a diagnosis of paracetamol overdose would have been really helpful. It would have been worth noting that acetylcysteine most efficacious within 8 to 10 hours after ingestion of paracetamol.
IV alpha1-antiprotease

A superficial talk about an important contributor to malabsorption. I think it would have been good to discuss why we need to assess for the prescence of IgA to begin with. What the role of HLA is in the disease. Moreover, the importance of biopsy as well as the long term complication of celiac disease include osteoprosis, neurological impairment and enteropathy associated T-cell lymphoma.

I think the lectures provide a quick summary of these sections. However, I don't think it is adequate depth particularly when it comes to management. This is exemplified by the management of oesophageal varices where it is unclear which management approach will be utilised first/preferred in the case of acute bleed nor is it clear when you will need to utilise TIPSS. Finally a number of diagnostic tests are mentioned throughout the talks but the audience is not provided with a basis to approach the patients sitting on front of them. As an example if a patient complains of dysphagia or dyspepsia what will be your first diagnostic tests and the order in which you will perform these test. I think this the higher level thinking required for step 2

So far the path lectures on my iPhone are great. I drive 30 minutes to school each way and I think they are very comprehensible even when it's this more passive learning style. They are perfect for getting my brain ready for school and winding down on the drive back.
