CNS Infections - Clinical Neurology

by Carlo Raj, MD

Central nervous system (CNS) infections are among the most devastating infectious diseases worldwide, often resulting in medical emergencies that require prompt management. Many types of pathogens can lead to CNS infections, affecting either the meninges, cerebrospinal fluid (CSF) or parenchyma. Meningitis (infection of the CSF or surrounding meninges) classically presents clinically with fever, severe headache, and objective neck stiffness, though this triad is rarely seen in practice. CNS parenchymal infection is termed encephalitis, which can present along a spectrum that includes symptoms such as fever, seizure, and general neurologic or psychiatric impairment. Focal CNS infections, such as abscesses, are more often seen in immunocompromised individuals. Each of these infection types requires a high degree of clinical suspicion and appropriate confirmatory tests to diagnose, and should be treated promptly to prevent significant morbidity and mortality.
