00:01 Our topic here is metastasis to ovaries tumor.

00:05 So what does that mean to you? That means that, say for example, I'll give you one big one.

00:09 Let's say that you have a diffuse type of gastric adenocarcinoma, a diffuse type of gastric adenocarcinoma.

00:19 Why did I say diffuse? Because it loves to spread.

00:22 Molecularly, would you tell me what this cancer is missing, so that it loves to spread? The E-cadherin is negative.

00:32 And think about all the different places that the diffuse gastric adenocarcinoma spreads.

00:36 which are now your left superclavicular node, your Leser-Trelat sign, and there's something called linitis plastica.

00:44 And it loves to spread down to ovary.

00:46 We then call this what? Krukenberg.

    00:48 Let's begin.

    00:49 Overall frequency, 5%.

    00:50 Malignancy, 5%.

    00:52 All groups are affected.

    00:54 Metastasis to the ovaries.

00:58 Morphology: Usually from other sites or organ.

01:03 Can be from the breast or GI tract.

01:05 I told you about the GI, do not forget about the breast.

01:08 And remember, association with pseudomyxoma peritonei with appendiceal tumor.

    01:13 Yeah, that's important.

01:15 What does pseudomyxoma peritonei mean to you? That means you have more or less malignant ascites and do not forget that.

    01:25 Krukenberg tumor.

    01:27 Bilateral mestastasis.

    01:29 Bilateral mestastasis from GI.

    01:32 Both ovaries.

01:33 Characterized by hematogenous spread from your diffused gastric adenocarcinoma to ovary.

01:40 Upon histologic examination, you'd find these Signet ring cancer cells or Signet ring cells.

    01:46 Important.

01:47 Why? It should've been in the stomach, but now it's spread down and metastasized to the ovary.

01:53 The site of origin is usually going to be the stomach.
