Most headaches are benign, and most patients can be safely treated and discharged with minimal workup. However, many serious and life-threatening conditions may present with headaches. Recognition, evaluation, and appropriate management of these dangerous secondary headaches are critical for preventing long-term disability or death. A primary physician’s role is to look for red flags—signs and symptoms from the history and physical that would point to a more serious underlying pathology. The emergency provider must decide which patients require immediate further testing, such as laboratory studies and imaging, to confirm the diagnosis and to direct management. In addition to diagnosing and managing life-threatening causes of a headache, the physician must be well versed in treating common primary headache disorders, as proper management of the natural course of the disease may improve patient outcomes.
