Emergency Medicine: General Topics

by Julianna Jung, MD, FACEP

Possibly one of the most stressful situations a young physician can imagine is being the first on the scene in an acute patient emergency. It can feel as though all of the medical knowledge you have amassed over the years has dissipated in an instant, and without a sound scaffold to rely on, it is easy to get lost in the chaos surrounding you. For this reason, it is important to develop a systematic approach to the acutely dying patient, starting with basic emergency management (e.g., airway, breathing, and circulatory assessments) and quickly brainstorming a differential diagnosis that includes the most likely causes of decompensation and those that are the most likely to kill the patient. Once this framework is built, a targeted history, examination, and diagnostic testing can be completed rapidly. If you are still early enough in training, this framework will also be invaluable for crafting a concise and informative patient presentation.
